Dean's Certification Form

To authorize the OSCCS at Duke University to release a student’s disciplinary record to an institution or third party, please complete the Authorization to Release form. OSCCS will only send a student Dean's Certification Form (disciplinary history forms) directly to the requested designated institution or third party.
